Faith in Action of the San Gorgonio Pass (FIASGP) recently announced it has been awarded a grant from the California Wellness Foundation to launch a mobile outreach initiative that expands Medi-Cal enrollment and community health access for underserved residents throughout the San Gorgonio Pass.

Banning, Beaumont, and Cabazon are among the most economically disadvantaged areas of Riverside County, with high unemployment, low insurance coverage and limited access to healthcare. The region’s predominantly Hispanic, rural population faces signifcant barriers to preventive care and behavioral health services.

Both Inland Empire Health Plan (IEHP) and Molina Healthcare have identified the San Gorgonio Pass as a priority region for Medi-Cal outreach due to persistently low enrollment.

Project overview With support from Cal Wellness, FIASGP will deploy a mobile outreach team consisting of Community Health Worker(CHW) and Peer Specialist. Operating from a mobile office vehicle equipped with a wheelchair lift and enrollment tools.

They expect to contact 600 individuals over the course of two years. Additionally they hope to conduct 200 screenings and assessments, 150 enrollments in Medi-Cal, ECM, or Community Supports and 100 connections to primary care or housing services within two years.

Advancing health equity is project will bring enrollment assistance and care navigation directly to rural neighborhoods, improving access to healthcare, reducing disparities, and strengthening service coordination for the region’s most vulnerable residents.
