Many people assume that a “normal” Vitamin B12 lab report guarantees good health. However, Apollo Delhi surgeon Dr. Anshuman Kaushal, popularly known on social media as The Angry Doc, has warned that normal test results can be misleading. Individuals may still experience persistent fatigue, tingling in the legs, forgetfulness, and irritability even when their B12 levels appear within the normal range.Functional B12 Deficiency: When Lab Reports DeceiveDr. Kaushal explained in a social media video, “Have you ever met someone who is always tired, forgetful, and depressed, but their report shows normal B12? Congratulations. You have just diagnosed functional B12 deficiency.” He added that this occurs when cells cannot use B12 properly, even if blood levels appear adequate.
He further illustrated the issue, saying, “Perfect B12 levels on paper may hide total cellular bankruptcy. It’s like having money in your account but no ATM card. Looks rich, feels broke.”
Why Standard Tests May Not Be EnoughAccording to Dr. Kaushal, many laboratories stop at measuring serum B12, which does not reflect cellular-level deficiency. He emphasized, “B12 and folate work like Batman and Robin. They repair DNA, produce red blood cells, and keep neurons healthy. Skip one, and your brain goes into ‘Gotham mode’—anxiety, brain fog, numb hands, tingling legs are all free bonus features.”
Who Is at RiskCertain groups are more vulnerable. People taking metformin or proton pump inhibitors, those on a vegan diet, and individuals who have undergone bariatric surgery are already in the “danger zone,” Dr. Kaushal warned.
He advised, “If you have symptoms despite normal B12 levels, discuss with your doctor and consider testing MMA, homocysteine, or active B12. Oral tablets may not work; you may need injections. Absorption is the villain, not the vitamin.”
Dr. Kaushal concluded, “Functional deficiency means your cells aren’t using B12 properly. Health is not just about numbers; it’s about function. Save your neurons and please don’t rely on B12 gummies for energy. This is biochemistry, not Bollywood fiction.”
